ABOUT THE SERVICE

The clerical or typographical errors which are covered under R.A. No. 9048 and R.A. 10172 are limited to those mistakes committed in the performance of clerical work in writing, copying, transcribing or typing an entry in the civil register that are harmless and innocuous, which are visible to the yes or obvious to the understanding, and can be corrected or changed only by reference to other existing record or records

 

CLIENT GROUP

The petitioner is a natural person filing the petition for the correction of a clerical or typographical error, a petition for change of first name, or change of day, month and sex. She/he must be of legal age and must have direct and personal interest in the correction of clerical or typographical error in an entry, change of first name or nickname and/or change of day, month and sex in the civil register.

 

REQUIREMENTS                    

R.A. 9048 – Petition for Correction of Clerical Error in the Certificate of Live Birth, Marriage and Death

  • PSA copy of document to be corrected
  • Baptismal Certificate
  • School records (Diploma, Transcript, etc.)
  • Marriage Contract, if applicable
  • Birth Certificate of children, if any
  • Birth Certificate of siblings
  • Birth Certificate of mother/father
  • Affidavit of Discrepancy

 

R.A. 9048 – Petition for Change of First Name

  • Birth Certificate (PSA Copy)
  • Baptismal Certificate
  • School records (Diploma, Transcript, etc.)
  • Marriage Contract, if applicable
  • Birth Certificate of children, if any
  • Affidavit of Discrepancy
  • Barangay, Police, NBI Clearance
  • Affidavit of Unemployment, if unemployed
  • Certificate from employer stating there is no administrative or any pending case whatsoever
  • Publication for two (2) consecutive weeks in newspaper of national circulation (after filing of petition)

 

R.A. 10172 – Petition for Correction of Clerical Error on Day, Month and Sex in the COLB

  • Birth Certificate (PSA and LCRO copy)
  • Baptismal Certificate
  • Earliest School records (Elem. School records Grade 1-6, if school doesn’t exist, certification from DECS with birthday and sex)
  • Marriage Contract, if applicable
  • Medical Records (laboratory, dental etc., if the owner has no medical records, an affidavit attesting to the facts shall be submitted)
  • Certification/Records from hospital/clinic where the owner was born with birthday and sex
  • Certification issued by accredited govt. physician attesting that the petitioner/document owner has not undergone sex change or sex transplant
  • Affidavit of Discrepancy
  • Barangay, Police, NBI Clearance
  • Certificate from employer stating that there is no administrative or any pending case whatsoever, in case the owner is not employed, an affidavit of non-employment shall be submitted
  • Publication for two (2) consecutive weeks in newspaper of national circulation (after filing the petition)

Return to the Office of the Municipal Civil Registrar and present the OR Informs the owner/petitioner that the process will take 2 to 3 months and how they will be contact once the submitted petition is approve by the PSA 3 minutes Assistant Registration Officer/
